NICE on DES 2008

On Friday 25th January 2008, BCIS and the British Cardiovascular Society received the draft of the NICE Final Appraisal Determination (FAD). This demonstrated that the Advisory Committee had paid heed to the clinical experts’ advice and alternative economic models to the one they had commissioned. Their recommendation is that drug-eluting stents can be used for lesions >15mm or in vessels <3mm (i.e. the same as the previous guidance) but only as long as the average price difference between drug-eluting stents and bare metal stents is less than £300.
The Advisory Committee rejected the BCIS request that diabetes was added to the above two criteria as an independent factor in choosing a DES. BCIS believes that the Committee’s reason for doing this was not based on scientific principles. BCIS also believes that the scientific basis for the latter part of the guidance is fundamentally flawed, and although this is more of an issue for industry, it will clearly affect local tendering exercises. Moreover, it may have consequences for future NICE evaluations for the technologies we use in our patients. We now have to decide whether to make an appeal against the FAD.

Change in CCAD Dataset

I am writing to let you know that there will be a minor change to the current BCIS-CCAD dataset (version 5.2.3). As you know I have resisted changing the dataset for as long as possible to allow units to get up to speed with collecting the data, without having to chase moving goalposts. However the problem is that this dataset does not allow us to measure time delays involved in moving patients between hospitals (particularly from non-PCI centres to PCI centres). Details of this change can be found on the PCI Data Collection page of the website.

BCIS Audit 2006

During the BCIS Autumn Meeting held in Dublin on September 21st, 2007, Peter Ludman presented the UK National PCI Audit data for 2006. As many in the field anticipated, the double digit annual growth in activity observed for the last 15 years has slowed. In 2006, there was a modest 4.3% increase in activity compared with the previous year to 73,612 PCI procedures (1216/million population) undertaken by 91 PCI centres in the UK, 95% of which are now CCAD compliant. Outcomes are similar to previous years with an overall procedure related hospital mortality of 0.74% (range 0 – 3.0%): case-mix, particularly the presence of cardiogenic shock, is the main driver of early mortality.
